Prepare for your assessment at clinic.

Please be aware that the service is for Birmingham residents and no assessment or recommendations can be completed for residents with a usual address outside the Local Authority, thank you.

Please bring along documents for the assessor to note such as:

Prescription/ list of medications

Letters which detail any diagnosis or health conditions

Photos of relevant parts of the property you wish to discuss

Complete the pre-assessment questionnaire if you have received one in the post.

 

Please do not attend clinic if you have an urgent need.

Will everyone who attends be seen at Clinic?

Not necessarily no. There may be times when OT Clinics are very busy and not everyone can be seen, even if arriving before 2pm, there may be the possibility that, in these circumstances the Clinic would be at capacity and you would be asked to attend on another day, at a different Clinic or to call the Contact Centre.

Unforeseen Clinic closures

Some venues may be closed at short notice meaning clinics will not take place on occasion.

Due to adverse weather conditions clinics may close at short notice. Please check here for updates on clinic closures. 

 

Special OT Clinics with the Alzheimer's Society.

Our friends and colleagues from the Alzheimer's Society will be alongside the Occupational Therapy Clinic team to provide support and advice for visitors to the SURC - Sutton United Reformed Church. OT Clinic will take place as usual.

A representative from the Alzheimer's Society will be joining us and visitors to the Sutton United Reformed Church or to the OT Clinic are welcome to drop in, they are there for anyone affected by dementia and will do what they can to help those affected to stay connected to their lives and the people who matter most to them, giving vital support face to face in a community setting.

No appointment is necessary, please drop in.
