Helping you to stay independent
Our aim is to prevent or delay your care needs becoming more serious. We will help you access services to get back your skills and abilities.
When planning the support we will provide, we consider the support network you already have from family, friends, services, facilities, and resources available in the area. This could be for example, local voluntary and community groups.
We will help you:
- choose from a range of high quality services to suit your needs
- get the information and advice you need to make good decisions about care
Support available from EICT
A range of services is available to support you, tailored to your needs and the outcomes you want to achieve with the team.
You will receive a holistic assessment from one or more professionals in the EICT service. This may include occupational therapists, physiotherapists, or nurses. If care support is required, an Adult Social Care worker will also be involved. Based on your assessment, you may receive help from early intervention assistants or therapy support workers.
EICT aims to help you build independence and resilience. You will be actively involved in planning your support, with choice and control over your goals. If appropriate, occupational therapists and physiotherapists will work with you for a short period to help you reach those goals. This may include:
- equipment to support daily living
- exercise therapy to improve mobility and strength
- support with managing medication
A personalised plan will be created with you, and therapy support workers may visit to assist with exercises or confidence-building tasks.
If you do not achieve all your goals
If you have not achieved all your goals near the end of your short-term support form EICT, we will discuss with you whether to refer you to a longer-term therapy service in the community.
Continuing Healthcare Checklist (CHC)
EICT only provides short-term support. The Continuing Healthcare Checklist is a separate process.
