Services to support your recovery after you leave hospital

After you have left hospital, our Early Intervention Community Team (EICT) will support and guide you as much as we can whilst you are under our care. 

These web pages explain important information about the services you will receive. 

At first, you will receive a short-term service for up to 42 days if you are:

  • 17 years old or above
  • live within Birmingham and have a Birmingham GP
  • medically fit for discharge

The quick discharge service is a care provider that will help you to get home safely. You will then be transferred to a different short-term care provider within a few days.

A team of NHS and council health and social care professionals will visit you at home to plan how we can help you to be as safe and independent as possible at home. Your family, friends and carers can support you during this visit, if you wish. 

The care and support service that you receive will be based on the goals that are identified to increase your independence and improve your quality of life. 

If you decide that you would like to plan and arrange your own care and support, we will provide you with information on how to do this and the associated costs

If our assessments show that you are eligible, you will then be moved to a long-term support provider.

Ending the short-term service early

You must engage with us in a timely manner to ensure that your care and support needs are planned as early as possible. We may contact you or your representative to ensure this happens.

If we are unable to contact you, or have evidence of unreasonable behaviour, we are sorry that we will have to stop providing the service.

If you need someone to explain om a different language

If you need someone to explain information in your own language when we visit you, but do not have a family member or friend who can do this, we can arrange for a free interpreter to attend.
