Eligibility

This service is provided to service users who are:

Aged 18 years and above

Birmingham Residents

Registered with a GP Practice in Birmingham or within Sandwell and West Birmingham.

Under the care of secondary mental health services, on the GP Serious Mental Illness register or have a long term mental health condition of 12 months or more.

You must complete a referral form on the below link before attending a session.
