Home from Hospital Care (HFHC) is a Birmingham-based Charity, registered in 1986. We provide a comprehensive six-week aftercare support service for Birmingham residents who are over 18 and are being discharged from hospital. The majority of our service users are older adults and live alone. All of the support that we provide is free and usually lasts for 6 weeks.
Our 15 dedicated staff and 32 enthusiastic volunteers provide;

• food parcels and toiletry packs on discharge
• white goods, clothing and essential items
• tablets and smart phones, with training, to help people stay in touch with loved ones
• a handyman service to help set up downstairs living and make adaptations at home
• a shopping service
• benefits advice and support
• transport to hospital appointments and support to collect prescriptions
• a befriending service
• a sitting-in and information and support service for carers
• support with bills and correspondence
• advice, information, signposting and referrals to other organisations.
