Social prescriber drop-in service at Age Concern Moorfield Hall - Friday's 1.30-4.30pm 

Many things can affect a person’s health and wellbeing such as stress, feeling lonely, isolation, finances, work, long term health conditions, unemployment and  lifestyle or other social factors. 
Social prescribing starts with a conversation, the link worker is there to listen to the person and put them in touch with the people or activities that might make them feel better.

Social prescribing can help you to have more control over your own health and find ways to improve how you feel in a way that suits you.
