Service 2 – Holistic assessments for deaf & hard or hearing individuals

This activity focuses on overing a portable, strengths based, holistic assessment plan, where staff will work 1:1 with clients to help them highlight and capture a clear picture of their aims, goals, strengths and needs, including finance, education and training, health and wellbeing, housing, and life skills; with the aim to support clients with coping strategies.
Individual would be able to then keep their own assessment and carry out actions which they can discuss with staff. This will prevent the individual having to re-tell their story to someone new and can aid clients in keeping track of their progress.
There is no associated fee with this service. Clients who are interested should contact the named staff member for further information.
